In order for a business to provide PHS to its employees, it needs a minimum of 10 employees earning more than $25k per year.
If your employee leaves, they take their plan with them. It is up to them to contribute to the plan, but the assets they created are theirs to use at their own discretion.
Yes. Each location, if they have a separate EIN number will need to apply and implement separately.
No. Even part time employees can qualify as long as they earn more than $25k per year.
PHS is simply a supplemental preventative health program. Employees will receive a significant reward for participating. In addition, their medical costs will be reduced as PHS can provide many free resources that would otherwise cost employees for provider visits and other care options.

Typically it will take about 4-6 weeks from the time payroll reports are received to actual implementation.
